Squad 8 Volleyball Open Play Waiver

Acknowledgment of Risk and Release of Liability

I, the undersigned participant, understand and acknowledge that participation in indoor recreational volleyball involves inherent and unpredictable risks. These risks include, but are not limited to, slips and falls; collisions with other participants; contact with the floor, net, ball, or surrounding equipment; overexertion; and injuries such as sprains, strains, fractures, concussions, or other serious injury. I understand that risks may also arise from the actions, omissions, or negligence of other participants.

I understand that Shirley and Andrew, organizers of Squad 8 Open Play, are not medical professionals and do not provide medical advice, diagnosis, or treatment. I voluntarily choose to participate and assume full responsibility for any injury, illness, loss, or damage that may occur in connection with my participation.

Equipment and Facility Use

I agree to:

• Treat all equipment and property with respect. I understand that the gym and equipment are rented from an outside organization, and I am responsible for any damage I cause.
• Follow all facility rules, including vacating the gym promptly at the end of the scheduled session, keeping the facility clean, and respecting areas of the school not designated for rental use.
• Use appropriate footwear and behave in a safe, respectful manner. I will follow reasonable instructions given by organizers regarding court safety, play format, and facility policies.
• Bring my own personal equipment when required and properly dispose of all trash and belongings. I will promptly report any unsafe condition or damaged equipment that I notice.

Health Acknowledgment

I certify that I am physically able to participate and that I do not have any condition (medical, physical, or otherwise) that would make participation unsafe. I understand it is my responsibility to warm up appropriately, play within my limits, and stop participating if I feel pain, dizziness, or discomfort.

In the event of an emergency, I authorize the organizers to seek emergency assistance on my behalf, and I understand that I am responsible for any resulting costs of care or transportation.

Indemnification

I agree to indemnify and hold harmless Squad 8, its organizers (Shirley and Andrew), agents, volunteers, and the property owner from any claims, damages, losses, or liabilities (including reasonable attorneys’ fees) arising out of or related to my participation in this activity, including injury, illness, death, or property damage.

Release of Liability

To the fullest extent permitted by law, by signing below I voluntarily release and forever discharge Squad 8, Shirley, Andrew, and the property owner from any and all liability, claims, demands, or causes of action, whether known or unknown, that arise out of or relate to my participation in the Open Play event.

Acknowledgment of Terms

I have read this Waiver and Release of Liability carefully, understand its contents, and agree to be bound by its terms. I understand that my typed name constitutes my electronic signature.
